Bioidentical Hormone Replacement: A Comprehensive Guide

Gabriel
October 23, 2026
8 min read

Bioidentical hormone replacement therapy, or BHRT, is one of the most searched and most confusing topics in menopause care. The word "bioidentical" gets used for two different things: FDA-approved estradiol and progesterone that most pharmacies stock, and custom hormone blends mixed by a compounding pharmacy. They can share the same molecule, but not the same testing, labeling or oversight. This guide covers what each one is, what the evidence shows and how strong it is, what changed in the FDA's warnings in November 2025, what it costs, and what to ask a licensed clinician before you start.

What "Bioidentical" Actually Means

Bioidentical hormones have the same chemical structure as the hormones your ovaries made before menopause, mainly estradiol and progesterone. The Menopause Society notes that the term began as a marketing term for custom-compounded hormones, but most people now use it for any hormone with the same structure as the body's own.

That structure is what sets them apart from the hormones tested in the Women's Health Initiative (WHI), the large trial that made many women wary of hormone therapy after 2002. The WHI tested one estrogen, conjugated equine estrogens taken by mouth, alone or with medroxyprogesterone acetate, a synthetic progestin. The Menopause Society's 2022 position statement points out that the WHI studied just one formulation and one route, so its results can't be applied to every product.

So "bioidentical" describes the molecule. It doesn't tell you whether a product is FDA-approved, how it was made, or whether it is safe for you.

FDA-Approved Bioidentical Hormones vs Compounded BHRT

Many bioidentical hormones are FDA-approved. The Menopause Society lists estradiol, estrone and micronized progesterone among them, and notes they are regulated and monitored for purity and come with package inserts that spell out the risks. They come in several forms:

  • Estradiol: pills, skin patches, gels and sprays for the whole body, plus vaginal creams, inserts and rings for local symptoms.
  • Micronized progesterone: oral capsules, taken with estrogen by women who still have a uterus to protect its lining.
  • Estradiol with progesterone: a combined daily capsule.

Compounded bioidentical hormones are mixed by a compounding pharmacist from a prescriber's order. The Menopause Society says they may combine several hormones, such as estradiol, estrone, estriol, DHEA, testosterone and progesterone, in untested combinations or routes such as pellets under the skin, implants and troches, which dissolve in the mouth. ACOG says customized compounded hormones pose more risk because they vary in strength and purity, so you can take too little or too much without knowing it.

Compounded medications aren't FDA-approved. A licensed clinician decides what's right for you.

People choose compounding for real reasons. Some need a dose or form no approved product offers. Some are allergic to an ingredient: brand micronized progesterone capsules contain peanut oil, and the FDA label rules them out for people with a peanut allergy. Others prefer a practitioner's custom approach. A 2020 National Academies report, done for the FDA, said compounded hormones should be limited to two situations: an allergy to an ingredient in an approved product, or a needed dosage form no approved product provides.

What the Evidence Shows, by Strength

Strong: hormone therapy works for menopause symptoms. The Menopause Society calls hormone therapy the most effective treatment for hot flashes and night sweats and for the vaginal and urinary symptoms of menopause, and says it has been shown to protect against bone loss and fractures. For healthy women under 60 or within 10 years of menopause, it says the benefits for bothersome hot flashes generally outweigh the risks.

Strong, and against the marketing: compounded isn't proven safer or better. The Menopause Society says custom-compounded hormones are not safer or more effective than approved bioidentical hormones, and that they may not even contain the prescribed amount. ACOG says there is no scientific evidence that compounded hormones are safer or more effective than standard hormone therapy, and it recommends FDA-approved hormone therapy over compounded. The National Academies found insufficient evidence to support the clinical use of compounded bioidentical hormones and said claims of superior safety were not backed by well-designed studies.

Mixed: route and type of progestogen. The Menopause Society's 2022 statement says limited observational data suggest less blood clot risk with skin patches and gels than with estrogen pills, and that micronized progesterone may be less likely to cause clots than other progestins. Some, but not all, observational studies link micronized progesterone with less breast cancer risk than synthetic progestins. The statement is clear that randomized trials comparing these options have not been done, so these are reasonable hopes, not settled facts.

Saliva Tests, Hormone Panels and Pellets

Some practitioners who prescribe compounded BHRT set and adjust doses using saliva, urine or blood hormone panels. The Menopause Society's 2022 statement says salivary and urine testing to set doses is unreliable and not recommended, because hormone levels shift through the day and absorption differs from person to person, and that blood hormone testing is rarely needed. If a practitioner suggests a panel, it's fair to ask how the result will change your plan.

Pellets are small compounded implants placed under the skin in an office. They are a different case from creams or capsules:

  • The National Academies said hormone pellets should be considered for the FDA's list of drugs that are difficult to compound, citing the complex delivery and a lack of required testing.
  • ACOG lists safety concerns about pellet therapy.
  • Aetna's coverage policy calls implantable estradiol pellets experimental because they have been shown to produce unpredictable, fluctuating estrogen levels.

If you're considering pellets, ask how the dose would be adjusted once one is in, and what each placement costs.

Safety and the 2025 Boxed Warning Change

On November 10, 2025, the FDA asked makers of menopausal hormone therapy to remove heart disease, breast cancer and probable dementia language from the boxed warning, and to drop the old advice to use the lowest dose for the shortest time. The boxed warning for endometrial (uterine) cancer stays on systemic estrogen-alone products. Heart disease and breast cancer warnings stay in the rest of the label for systemic products, and updated labels add that starting therapy can be considered for moderate to severe hot flashes before age 60 or within 10 years of menopause. By February 12, 2026, the FDA had listed the first six products with updated labels.

Those labels cover FDA-approved products. Compounded hormones have no FDA label at all, and The Menopause Society lists the missing label outlining risks among its safety concerns. "Bioidentical" does not mean risk-free.

ACOG says systemic hormone therapy usually isn't recommended if you have ever had:

  • Breast or endometrial cancer
  • A stroke or heart attack
  • Blood clots
  • Liver disease

It isn't for pregnancy. If you still have a uterus, estrogen needs a progestogen alongside it, because estrogen alone raises the risk of overgrowth of the uterine lining. ACOG notes a small risk of stroke and blood clots with hormone therapy that rises with age, and says patches, sprays and rings may pose less risk than pills.

Call your clinician about any vaginal bleeding after menopause. Call 911 for chest pain, sudden shortness of breath, a painful swollen leg, or signs of a stroke such as face drooping, arm weakness or trouble speaking.

What BHRT Costs

FDA-approved bioidentical hormones are among the cheapest menopause medicines. In CMS's national pharmacy cost survey for October 2026, summarized in Gabriel's HRT cost guide (askgabriel.com/guides/hrt-cost), a month of generic estradiol pills cost pharmacies about $2, generic estradiol patches about $49 to $70, and generic micronized progesterone about $7 to $12. Your cash price adds the pharmacy's own fees.

Compounded hormones have no national price survey, so each pharmacy or clinic sets its own price, and pellets add an office visit each time they are placed. Insurance tends to follow FDA approval: in CMS's 2026 Medicare drug plan data, generic estradiol pills are on every plan's drug list, while insurers such as Aetna and Cigna classify compounded pellets as experimental.

Through Gabriel, a free account pays cost-plus prices on prescriptions, and a licensed clinician reviews every prescription request and decides.

How to Decide, and What to Ask

Integrative practitioners who prescribe BHRT often build it into a wider plan for sleep, stress, strength training and nutrition, and some prefer compounded formulas for their flexibility. Conventional menopause specialists usually start with FDA-approved estradiol and micronized progesterone. Either way, the choice is yours to make with a licensed clinician. Questions worth asking:

  1. Do my symptoms and history make me a candidate for systemic hormone therapy, vaginal estrogen only, or a nonhormonal option?
  2. Is there an FDA-approved bioidentical product that fits me? If you suggest a compounded one, why?
  3. Patch, gel, spray or pill: what's the clot and stroke trade-off for me?
  4. If I have a uterus, which progestogen will protect the lining, and what bleeding should I expect?
  5. Do I need any hormone tests, and how would a result change my dose?
  6. How will we follow up, and when will we review whether to continue?

Bioidentical hormones are not a fringe idea. FDA-approved estradiol and micronized progesterone are bioidentical, well studied and low-cost, and The Menopause Society calls hormone therapy the most effective option for hot flashes. Custom-compounded versions have a place when someone has an allergy or needs a dose no approved product offers, but The Menopause Society, ACOG and the National Academies all say they haven't been shown to be safer or more effective, and they come without an FDA label. Whatever you lean toward, bring your full history to a licensed clinician, ask the questions above, and make the call that fits your body and your goals.
