• Ketamine therapy / ketamine-assisted therapy

  Ketamine infusions, esketamine (Spravato), at-home ketamine through telehealth, and ketamine-assisted therapy, prescribed and overseen by licensed clinicians.

• Psilocybin sessions (state-licensed programs)

  Oregon psilocybin service centers and Colorado healing centers, with licensed facilitators. New Mexico’s medical psilocybin program is set to open by the end of 2026.

• MDMA-assisted therapy (not yet available)

  The FDA declined approval in 2024 and a resubmitted application is pending. For now, access is limited to clinical trials and expanded access.

• Psychedelic integration sessions

  Preparation and integration sessions, in person or by telehealth.

• Ibogaine therapy (not yet available)

  No licensed US treatment program yet. Colorado has authorized an ibogaine research pilot.

Claims that don’t meet our coverage criteria.

• Services from non-licensed or non-certified practitioners

  Treatments from providers without proper credentials or certifications.

• Claims exceeding monthly benefit cap

  Reimbursement requests that go over your plan’s monthly limit.

• Services received before membership start date

  Treatments obtained before your Gabriel membership became active.

• Services received during waiting period

  Any waiting period in your active Benefits Schedule must be satisfied. Check your schedule and pre-approval requirements.

• Claims submitted outside applicable submission deadlines

  Submit requests on or before your membership end date. After your membership ends, new requests can’t be submitted, including for care you received while you were a member.

• Duplicate claims for the same service/date

  Multiple reimbursement requests for the same treatment on the same day.

• Claims without a valid receipt or invoice

  Reimbursement requests missing required documentation or proof of payment.

• Services outside the United States

  Treatments received in other countries (except where explicitly covered).
