The steps
- Get your labs. Two fasting morning total testosterone tests on different days, plus tests your clinician may want such as free testosterone, LH, FSH, hematocrit and PSA. See total testosterone and free testosterone.
- Have a telehealth visit. The clinician reviews symptoms, health history, medications and fertility plans.
- The clinician decides. A diagnosis needs symptoms plus consistently low levels.
- Prescription to a licensed pharmacy. Your clinician chooses the form and dose.
- Monitoring. Testosterone, hematocrit, PSA and blood pressure checks, mostly in the first year.
What makes online TRT legal
- A prescription. Testosterone is a controlled substance. Prescriptions can be refilled only a limited number of times.
- A telehealth visit that meets federal rules. The DEA and HHS extended telemedicine prescribing of controlled medications without a prior in-person evaluation through December 31, 2026. After that, a permanent rule may add new requirements.
- A licensed pharmacy. A safe pharmacy requires a prescription, is licensed by a state board of pharmacy, and has a U.S. address and a pharmacist you can reach.
Red flags with online TRT
- No lab tests, or only one test drawn at any time of day.
- A prescription promised before anyone reviews your results.
- No discussion of fertility, hematocrit or prostate checks.
- No plan for follow-up labs.
- Testosterone sold without a prescription, or labeled for research. That's illegal, and quality is unknown.
The Endocrine Society advises against starting TRT with untreated severe sleep apnea, a hematocrit above 48%, prostate or breast cancer, uncontrolled heart failure, or a heart attack or stroke in the past 6 months. A careful service asks about these.
Before you start: fertility
TRT lowers sperm production and can cause infertility. If you want biological children soon, tell your clinician first. See TRT side effects.
How Gabriel helps
Gabriel can help you find and book a practitioner who manages testosterone therapy. A free Gabriel account pays cost-plus prices on lab tests, and you can upload any lab report for a plain-words explanation before your visit. See the comprehensive hormone panel.
Common questions
Is online TRT legal?
Yes, when a licensed clinician evaluates you and prescribes it, and a licensed pharmacy fills it. Federal telemedicine flexibilities for controlled medications run through December 31, 2026.
Do I need lab tests for online TRT?
Yes. Diagnosis needs consistently low fasting morning testosterone on two tests, plus symptoms. Safety labs like hematocrit and PSA come before starting.
Do I need an in-person visit for TRT?
Not through December 31, 2026, under the current federal telemedicine extension. Check for changes after that date.
Can I buy testosterone online without a prescription?
Not legally. Testosterone is a controlled substance, and products sold without a prescription may be fake or contaminated.
How often do I need blood tests on TRT?
Several times in the first year for testosterone, hematocrit and PSA, then less often. Your clinician sets the schedule.
