40Hz Gamma Entrainment Therapy.

What it is

40Hz gamma entrainment uses light and sound that pulse 40 times a second to encourage the brain's gamma rhythm, which is linked to memory and attention. Research led by Li-Huei Tsai at MIT's Picower Institute found that in mice, it activated the brain's immune cells (microglia) and reduced amyloid and tau, hallmarks of Alzheimer's.

In small early trials in people with Alzheimer's, daily use was well tolerated and linked to slower brain shrinkage and less decline on some measures. A larger phase 3 trial of a device from Cognito Therapeutics is underway to test whether it slows decline.

What happens at a visit

1. A baseline check of memory, thinking and neurological history.
2. Fitting a headset or setup with LED lights and speakers.
3. Light and sound pulsing together at 40Hz for 30 to 60 minutes.
4. Most people find the pulsing easy to tolerate, even relaxing.
5. Daily sessions, often for at least 6 to 8 weeks.
6. Regular memory and biomarker checks to track any change.

Evidence and safety

The evidence is promising but early: strong animal research, plus small human trials that found it well tolerated with encouraging signals in Alzheimer's. A larger phase 3 trial is underway, and the device being tested has an FDA designation meant to speed review, which is not an approval. Flickering light can trigger seizures in people with photosensitive epilepsy, so anyone with a seizure history should avoid it unless a neurologist approves. Use it alongside, not instead of, dementia care.
