Mounjaro vs Zepbound: why the brand matters
Tirzepatide is one medicine sold by Eli Lilly under two brand names. Plans decide coverage by the approved use and your diagnosis:
| Brand | Approved for | How often plans cover it |
|---|---|---|
| Mounjaro | Blood sugar in adults and children 10 and older with type 2 diabetes; lowering heart attack, stroke and death risk in adults with type 2 diabetes at high risk | Commonly covered for type 2 diabetes, often with prior authorization |
| Zepbound | Helping adults with obesity lose weight and keep it off, with a reduced-calorie diet and more physical activity | Less common; depends on your plan or employer |
| Zepbound | Moderate to severe obstructive sleep apnea in adults with obesity (FDA approval December 2024) | Varies; Medicare says Part D plans may cover it |
Lilly says plainly that Mounjaro is not a weight loss drug. If you don't have type 2 diabetes, ask your plan about Zepbound instead.
Coverage by type of insurance
- Employer plans. In KFF's 2025 survey, 43% of firms with 5,000 or more workers covered GLP-1 drugs for weight loss, versus 16% of firms with 200 to 999 workers. About a third of firms that cover them require a dietitian, case manager or lifestyle program first.
- Medicare. Part D plans may cover tirzepatide for type 2 diabetes or moderate to severe sleep apnea. For weight management, the temporary Medicare GLP-1 Bridge offers the Zepbound KwikPen for a $50 copay a month to eligible members, July 1, 2026 through December 31, 2027. See does Medicare cover GLP-1s.
- Medicaid. State programs must cover GLP-1s approved for type 2 diabetes; obesity coverage is optional, and KFF counted 13 states covering it as of January 2026.
Savings card if your plan covers it
If your commercial plan covers Zepbound, Lilly's savings card can bring a one-, two- or three-month fill of the single-dose pen to as little as $25, up to $100 in savings a month and $1,300 a year. If your commercial plan doesn't cover Zepbound, the card can lower the single-dose pen to as little as $499 a month.
The card can't be used with Medicare (including Part D and Medicare Advantage), Medicaid, TRICARE, VA or other government drug coverage. Lilly's card programs end December 31, 2026.
How to check your coverage
- Call the number on your insurance card or check your plan's drug list (formulary).
- Name the brand and your diagnosis: Mounjaro for type 2 diabetes, or Zepbound for weight management or sleep apnea.
- Ask what's required: prior authorization, a lifestyle program, quantity limits or trying another drug first.
- Ask which form is covered. Zepbound comes as a single-dose pen, a KwikPen and vials, and plans may cover only some forms.
- Ask what you'd pay: your copay or coinsurance and any deductible.
If a plan says no, ask for the reason in writing so your prescriber can respond. Coverage varies by plan, and drug lists can change each year.
If your plan doesn't cover tirzepatide
Lilly sells Zepbound vials and KwikPens for cash through LillyDirect for $299 to $449 a month with on-time refills, and Mounjaro for $499 a month (as of October 2026). See Zepbound cost without insurance.
Through Gabriel, compounded tirzepatide is $135 to $225 a month, depending on the dose, with a free account ($131 to $218 with Gabriel Core and $128 to $213 with Plus), if a licensed clinician approves your prescription request.
Compounded medications aren't FDA-approved. A licensed clinician decides what's right for you.
Common questions
Does insurance cover Zepbound for weight loss?
Sometimes. It depends on your plan and is more common at large employers. Most plans that cover it require prior authorization, and some require a lifestyle program.
Is Mounjaro covered by insurance?
For type 2 diabetes, usually yes, often with prior authorization. Mounjaro isn't approved for weight loss, so plans generally won't cover it without a type 2 diabetes diagnosis.
Will insurance cover Zepbound for sleep apnea?
Some plans do, even if they don't cover weight-loss drugs. Zepbound is FDA-approved for moderate to severe obstructive sleep apnea in adults with obesity, and Medicare says Part D plans may cover it for that use.
Does Medicare cover tirzepatide?
Part D plans may cover Mounjaro for type 2 diabetes and Zepbound for sleep apnea. For weight management, eligible members can get the Zepbound KwikPen for $50 a month through the Medicare GLP-1 Bridge through 2027.
How do I get prior authorization for Zepbound?
Your prescriber submits it to your plan with your diagnosis and health history. Ask your plan what it requires and how long a decision takes.
