Coverage depends on why it's prescribed
Semaglutide is one medicine sold under two brand names with different approved uses. Plans usually decide coverage by the approved use and your diagnosis:
| Use | Brand | How often plans cover it |
|---|---|---|
| Type 2 diabetes (blood sugar, plus heart and kidney risk in diabetes) | Ozempic | Commonly covered, usually with prior authorization or quantity limits |
| Weight management in adults with obesity, or overweight with a weight-related condition | Wegovy | Less common; depends on your plan or employer |
| Lowering heart attack and stroke risk in adults with heart disease and obesity or overweight | Wegovy | Varies; Medicare routes this use to Part D plans |
Medicare also says Part D plans may cover GLP-1s for moderate to severe sleep apnea and for a type of fatty liver disease (noncirrhotic MASH). Ask your plan whether that applies to you.
Coverage by type of insurance
- Employer plans. In KFF's 2025 survey, 43% of firms with 5,000 or more workers covered GLP-1s for weight loss, compared with 30% of firms with 1,000 to 4,999 workers and 16% of firms with 200 to 999. About a third of firms that cover them require a dietitian, case manager or lifestyle program first.
- Marketplace (ACA) plans. Weight-loss coverage is rare. KFF found every Marketplace plan that covered a GLP-1 approved for obesity required prior authorization.
- Medicare. Part D plans may cover semaglutide for type 2 diabetes, heart risk reduction, moderate to severe sleep apnea or noncirrhotic MASH. For weight management, the temporary Medicare GLP-1 Bridge offers Wegovy for a $50 copay a month to eligible members from July 1, 2026 through December 31, 2027. See does Medicare cover GLP-1s.
- Medicaid. State Medicaid programs must cover GLP-1s approved for type 2 diabetes. Coverage for obesity is optional: KFF counted 13 states covering it as of January 2026, usually with prior authorization.
Prior authorization and other requirements
Prior authorization means your prescriber has to show the plan that you meet its rules before it pays. Rules differ by plan. As one example, the Medicare GLP-1 Bridge asks prescribers to confirm BMI, certain health conditions and that the drug is part of a diet and exercise program. Plans may also set quantity limits.
If a plan denies coverage, ask why in writing. Your prescriber can submit more information or appeal. If your employer doesn't cover Wegovy, Novo Nordisk offers a sample letter your clinician can use to request coverage through HR.
How to check your coverage
- Call the number on your insurance card or check your plan's drug list (formulary).
- Ask about the specific brand and your diagnosis: "Is Ozempic (or Wegovy) covered for my condition?"
- Ask what's required: prior authorization, a lifestyle program, quantity limits or trying another drug first.
- Ask what you'd pay: your copay or coinsurance, and whether you have a deductible to meet first.
- If you have commercial insurance that covers it, ask your pharmacy about the manufacturer savings offer, which can lower the cost to as little as $25 a month. It can't be used with government plans.
Coverage varies by plan, and plans can change their drug lists each year.
If your plan doesn't cover semaglutide
Novo Nordisk sells Wegovy and Ozempic for cash through NovoCare Pharmacy, starting at $149 a month for the pills and $349 for the pens after a new-patient offer (as of October 2026). See Ozempic cost without insurance.
Through Gabriel, compounded semaglutide is $149 a month at every dose, if a licensed clinician approves your prescription request.
Compounded medications aren't FDA-approved. A licensed clinician decides what's right for you.
Common questions
Does insurance cover semaglutide for weight loss?
Sometimes. Coverage for weight management (Wegovy) depends on your plan and is more common at large employers. Most plans that cover it require prior authorization.
Is Ozempic covered by insurance for diabetes?
Usually. Most plans, including many Marketplace and Medicare Part D plans, cover Ozempic for type 2 diabetes, often with prior authorization or quantity limits.
Will insurance cover Ozempic if I don't have diabetes?
Usually not. Ozempic is approved for type 2 diabetes, and plans generally cover it for that diagnosis. For weight management, ask whether your plan covers Wegovy.
Does Medicare cover Wegovy?
For weight management, eligible Part D members can get Wegovy for a $50 monthly copay through the Medicare GLP-1 Bridge, July 2026 through 2027. Part D plans may also cover GLP-1s for heart risk reduction, sleep apnea or noncirrhotic MASH.
How do I get prior authorization for semaglutide?
Your prescriber submits it to your plan, usually with your diagnosis, BMI and related health history. Ask your plan which form it uses and how long a decision takes.
