Is TRT a steroid?
Testosterone is the body's main male sex hormone. Anabolic-androgenic steroids are synthetic substances similar to testosterone that promote muscle growth and male traits. Testosterone products themselves are also commonly misused. So TRT uses a steroid hormone, but using it to treat diagnosed low testosterone is not the same as steroid misuse.
Legally, testosterone is a controlled substance in the United States. It needs a prescription, and refills are limited.
TRT vs anabolic steroid misuse
| TRT | Anabolic steroid misuse | |
|---|---|---|
| Purpose | Bring low testosterone back to normal in diagnosed hypogonadism | Build muscle, strength or appearance |
| Target level | Mid-normal testosterone | Well above normal |
| Dose | Set by a clinician | Often 10 to 100 times medical doses |
| Substances | FDA-approved testosterone products | Testosterone and synthetic steroids, often stacked |
| Monitoring | Testosterone, hematocrit, PSA, blood pressure | Usually none |
| Legal status | Legal with a prescription | Illegal without a prescription |
How the risks differ
Steroid misuse is linked to high blood pressure, blood clots, heart attack and stroke even in young athletes, liver damage and tumors, low sperm count, testicular shrinkage, breast growth in men that may be permanent, aggression, and depression after stopping.
TRT has real side effects too: higher hematocrit, higher blood pressure, acne, lower sperm count and worsened sleep apnea. Monitoring is meant to catch them early. In 2025, the FDA noted the TRAVERSE trial found no increase in major heart events in men treated for hypogonadism. See TRT side effects.
When TRT becomes misuse
MedlinePlus warns that taking more testosterone than prescribed, or using it without medical direction, can cause heart attack, heart failure, stroke, liver disease, seizures and serious mood changes. Cleveland Clinic advises against TRT for bodybuilding or to slow normal aging. Never change your dose on your own; your clinician decides the dose.

Common questions
Is TRT the same as taking steroids?
No. TRT restores low testosterone to normal with a prescription and monitoring. Steroid misuse uses much higher doses, often of several drugs, to build muscle.
Is TRT legal?
Yes, with a prescription from a licensed clinician for diagnosed low testosterone. Using testosterone or other anabolic steroids without a prescription is illegal.
Is testosterone a controlled substance?
Yes. It needs a prescription, and refills are limited.
Why do people misuse steroids?
Mostly to build muscle and change appearance, often at doses 10 to 100 times higher than medical doses. Those doses carry serious heart, liver, fertility and mood risks.
